Release – Nocturne Studios

AcknowledgementsTo the best of my knowledge, I do not have any physical, medical or mental impairments which might interfere with or affect my ability to knowingly and voluntarily decide to have tattoo-related work done at this time. I agree to follow all aftercare instructions regarding the care and maintenance of my tattoo while it is healing. I agree that any touch-up work needed, due to my failure to fully follow those instructions, will be done at my expense. I understand that a tattoo, like any other cut or abrasion, is susceptible to blood-borne pathogens and allergic reactions, and that any resulting problem may be a direct result of my failure to follow the instructions provided by my artist in regards to the care and maintenance of my tattoo. I understand that if my skin-tone is darker, the colors may not appear as bright as they would on fairer complexions. We do ask that you stop back into the shop, if at all possible, 3-4 weeks after you receive your tattoo so that your artist may see how it healed and determine if it requires any potential touch-up work. I HAVE HAD THE OPPORTUNITY TO DISCUSS THE RISKS WITH MY ARTIST AND VOLUNTARILY ACCEPT ANY AND ALL RISKS ABOVE.I acknowledge that my tattoo artist is an independent contractor, and NOT an employee of NOCTURNE STUDIOS. I hereby waive and release any and all claims against NOCTURNE STUDIOS, it’s principle agents and independent contractors arising out of, or relating to, any personal injury, property damage, allergic reaction, infection or death that may happen as a result of the tattoo work that I requested at, or through, NOCTURNE STUDIOS or any of its representatives.I agree to leave the premises of NOCTURNE STUDIOS, or any establishment where NOCTURNE STUDIOS is engaged in business, promptly upon request, for any reason whatsoever, by any representative of NOCTURNE STUDIOS. I agree to pay any and all damages and/or injuries to any persons or property belonging to NOCTURNE STUDIOS or any of its representatives or anyone who may become contractually liable due to my decision to have tattoo-related work done by NOCTURNE STUDIOS or any of its representatives.I agree that this Waiver and Release also pertains to and is designed to protect any and all establishments where NOCTURNE STUDIOS or its representatives conduct business.I understand that there will be NO CASH REFUNDS ON DEPOSITS, due to the artist’s time for designing and drawing my tattoo in addition to reserving time for my appointment. However, I can receive full value of my deposit in the form of a shop credit for later use, as long as I give a minimum of a 24 hour notice of cancellation/rescheduling.I understand and agree that this Waiver and Release is a legally binding document, intending to be legally bound. I hereby warrant to NOCTURNE STUDIOS that I have read and understand the above information and verify that all information provided by myself herein is accurate and true.
